  • Minor services: Up to 80% covered.

  • Cosmetic treatments: Usually not covered.

Your plan may come with other details as well, such as a deductible, a waiting period before you can receive coverage for certain services, and an annual maximum. All such stipulations can affect your out-of-pocket costs for any given treatment plan.
